Workout Contract Template:

-I understand that participation in this program carries some inherent risks including but not limited to injuries sustained while exercising. I hereby release [trainerufffds name/company name] from any liability arising out of or related to my participation in this program.

-I agree to follow all instructions given to me by my trainer during our sessions and I understand that failure to do so may result in injury. I also agree to listen to my body and stop exercising if I feel pain or discomfort at any time during our session.

-If for any reason I am unable to attend our scheduled session(s), I agree to notify my trainer at least 24 hours in advance so that we can reschedule for another time/day. If I do not show up for our scheduled session or cancel with less than 24 hours notice, I understand that I may be charged the full price for that session

What is included in the fitness class?

The fitness class includes a warm-up, cool-down, and various exercises in between. The number of repetitions and sets may vary depending on the class, but usually, there is a focus on cardiovascular endurance and strength training.

What is not included in the fitness class?

-You will not be able to use the class if you are injured or pregnant.

-The class does not include personal training, so if you want one-on-one attention, you’ll need to sign up for a separate package.

-The class also doesn’t include access to the gym’s other facilities, so if you want to use the sauna or steam room, you’ll need to purchase a separate pass.

Cancellation and refund policy

If you need to cancel or reschedule your session, please provide at least 24 hours notice. If you do not provide at least 24 hours notice, you will be charged the full session rate.

No refunds will be given for any reason once services have been rendered.

Liability waiver

In order to participate in our personal training program, you must sign a liability waiver. This waiver releases us from any liability arising out of your participation in the program, including but not limited to injuries sustained during workouts.

By signing this waiver, you acknowledge that you are participating in the program at your own risk and that you understand the risks involved. You also agree to abide by our rules and regulations.
